Juvederm, Restylane and Perlane are all excellent HA fillers that are very soft and moldable fillers, and since hyaluronic acid is naturally found in the body, HA’s have a very low rate of lumps and bumps. I always recommend an HA  to patients who have never had filler injections before, and here are the three reasons why:

Because HA’s are so soft, they can be molded into any shape you choose. I will often give my patients a hand mirror during the procedure to see whether or not they like the size and shape of their new cheeks, and if they want to make an adjustment, I can actually sculpt a different look.
Because hyaluronic acid is found naturally in the body, HA fillers tend to absorb quickly (4-6 months). While this sounds like a bad thing to you filler veterans out there, I think it’s a good thing for the newbies. If you’ve never had fillers before, there’s always a chance that you won’t be crazy about your new look, and if this happens, the HA fillers will take the least amount of time to dissolve.
The best reason to use an HA for first-timers is that they are reversible! That’s right… if you don’t like them, they can be reversed and you can go right back to your normal self in 24-48 hours. I have never actually had a patient ask for her cheeks to be put back to normal, but if it ever happens, it is possible to do as long as we used an HA.
#2 Radiesse
Radiesse is not an HA… it is a chemical called hydroxyapatite. It’s thicker and stiffer than the HA’s, but it also lasts longer… about twice as long actually. The major benefit of Radiesse is that it is fairly common for it to last 9-12 months (instead of 4-6 months like HA’s). However, Radiesse also has a few downsides:

Because Radiesse is thicker than JUVEDERM or Restylane, there is a slightly higher incidence of lumps and bumps. Lumps are extremely rare as long as your plastic surgeon is experienced with cheekbone enhancement, and even when they do occur, most of them can be massaged away in a day or two.
The most important thing to know about Radiesse is that it is not reversible. This is the main reason why I discourage filler-first-timers from going for Radiesse right off the bat. I totally understand that it sounds like the more attractive option since it lasts up to a year, but if you happen not to like the look of your new cheeks, you will be stuck with them for a year. If you’re a newbie, go for a “trial run” with JUVEDERM or Restylane, and then once you know you love it, switch to Radiesse the next time you have your cheeks done.
#3 Sculptra
Sculptra is favorite product for cheekbone augmentation. Sculptra is not a “filler” per se… it is a collagen stimulator, and it’s truly in a class by itself. It produces the softest, most natural, longest-lasting, results of any product or procedure, and it’s injectable of choice for cheekbones and under-eye rejuvenation. That being said, there are some important considerations with Sculptra:

With Sculptra, you won’t see your final result for at least 3-4 months. Because Sculptra is a collagen stimulator, you have to wait for your body to produce enough collagen to see a change. This gradual result is very attractive to some women who want to make sure that no one notices that they had anything done. In contrast, the results from JUVEDERM, Restylane and Radiesse can be seen immediately after the procedure.
Sculptra requires a series of 3-4 injections spaced about 4 weeks apart, so it’s a much more significant time commitment than a single injection session with another filler.
Because Sculptra is the most high-tech product available, and because it requires the most advanced injection technique, it’s more expensive. That being said, Sculptra is not necessarily more expensive in the long run because it lasts 4-5 times longer than JUVEDERM or Restylane, and 2-3 times longer than Radiesse. So, while the initial cost of Sculptra is more expensive, it’s quite a bit cheaper in the long term. And remember, you do get what you pay for, and the results I’ve seen with Sculptra are in a class by themselves just like the product is!
#4 Cheek Implants
Cheek implants are an option for any of the filler veterans out there who want a permanent solution for their cheeks. Cheek implants can be placed through tiny incisions inside the mouth, so there are no visible scars, and because the results are permanent, you won’t ever need an injectable again! In spite of these seemingly wonderful benefits, cheek implants have fallen out of favor slightly for a few reasons:

Surgery and anesthesia is required to place cheek implants, whereas filler injections can easily be performed in the office without anesthesia.
There is more downtime with cheek implants. While small bruises and slight swelling are possible after injectables, they are guaranteed after cheek implants. Your cheeks will be swollen, bruised, and tender for a week or so after surgery, and you will need to take at least a few days off of work. For some people, this is a minor price to pay for a permanent result, so it’s an entirely individual decision.
There is a slightly higher rate of asymmetry with cheek implants as compared to fillers, and this can sometimes require another operation if the asymmetry is noticeable enough. Filler asymmetry is much easier to correct, and is much less common.
At the end of the day, the most important decision is not which filler you choose, or whether you decide that you want cheek implants. The most important decision is who you choose to perform your procedure! Facial fillers replace skin substances that, when lost, cause wrinkles. The skin thins by loss of collagen and fluid. In youth, the skin remains plump from a natural sugar called hyaluronic acid that attracts and binds to the body’s water. With the advent of choices for facial fillers, we can choose the best for the specific problem and longevity.

There are now so many fillers available, we prefer to categorize them by their ideal usage. Fillers can be used to treat three conditions:

1. Superficial fine lines

2. Facial folds and crevasses

3. Facial volume loss

We all want fillers to last as long as possible, but don’t want bumps from over-injecting. Gradual, precise and pointed is our method. The most superficial lines, those that appear “etched” into the skin require the thinnest fillers. As the need for more volume replacement increases, fillers are placed deeper and can be thicker. Medium thickness fillers are used for actual folds in the skin and deeper crevasses, such as the naso-labial folds. Naso-labial folds run from the corner of nose to the corner of the mouth. Medium depth fillers also may be very effective for melo-labial folds that run downward from the corners of the mouth to the jaw; otherwise referred to as “marionette” lines. These thicker fillers are also longer lasting than the thin, superficial fillers.

Since there are a number of available fillers for fine and deep wrinkles, both may be used in the same patient, and in conjunction with one another. We often layer fillers in the same areas in order to plump folds and fill the lines that may lie in the depths of those folds.

The thickest fillers are more for volume enhancement for the face that needs to be “plumped up.” As the face ages, the cheeks get hollow in addition to droopy. Patients may need lifting as well as volume. In younger patients, however, facelifts can often be delayed by filling the cheeks. This can actually give a small lift as well and is called the “Liquid Facelift.” Of course, nothing substitutes for a beautiful and professional facelift if it’s a means to elevating jowls or excess skin under the neck. But we can go a long way in making you look more refreshed by starting with fillers.

There are a number of options for facial volume restoration. The most natural technique is using one’s own fat. The fat is re-injected immediately after removing it from elsewhere in the body. This technique is called lipo-transfer. However, we now use it less than we did a few years ago because lipo-transfer is similar to a surgical procedure in preparation and healing time. Plus there are a number of facial fillers available that can restore facial volume that may last for up to a year or more. These are Sculptra, Radiesse, and the thinker Hyaluronic Acid products; Perlane and Juvederm Ultra Plus.

There are many types of injectable fillers available today. The most popular fillers are made from hyaluronic acid (Restylane, Juvederm).
